Job Description:
Fully remote care coordination role at Talkspace supporting patients through activation and early engagement. High school diploma plus about 1-2 years of patient or customer support. Pays $20 to $25/hour.
About the Role
Talkspace is hiring a remote Care Coordinator on its Referral Management team. You will support patients throughout their care journey, monitor engagement, clear barriers to care, and coordinate with providers when patients need additional support.
What You Will Do
- Monitor patient lead-up requirements and paperwork so patients are ready for first appointments
- Follow up on outstanding requirements and identify barriers to timely activation
- Conduct outreach during the first 30 days of care to support engagement
- Follow up on missed appointments and help prevent incorrect billing for missed first visits
- Support escalation or step-up in care coordination with providers and internal teams
- Maintain accurate documentation and protect patient confidentiality
What You Need
- High school diploma or equivalent required; associate or bachelor's degree in healthcare, social services, or business preferred
- 1-2 years of experience in care coordination, patient support, customer support, or case management
- Experience working directly with patients or members preferred
- Strong organization and ability to manage multiple patients and priorities
- Remote work on Eastern or Central time
Location & Pay
