Call patients to explain advance care planning and schedule appointments, and handle inbound questions by phone, email, and text. Remote US role paying $24.52/hour; high school diploma required.
About the Role
Aledade supports independent primary care practices. The Outreach Specialist is the first step in the patient journey for the Comprehensive Advance Care Planning (CACP) program, handling high-volume calls and scheduling.
What You Will Do
- Make high-volume outbound calls to educate patients and schedule appointments
- Answer inbound patient questions and manage follow-ups
- Manage patient emails and text messages
- Follow scripts and SOPs and meet performance metrics
- Keep everything HIPAA compliant
What You Need
- High school diploma
- At least 1 year of call center outreach, customer service, or telesales experience
- Comfort with phone systems and CRM software
- Ability to work core hours (8 AM to 8 PM Eastern or local time)
- Reliable high-speed internet

Remote anywhere in the US. Pay is $24.52 per hour.
