Work-from-home patient outreach job with Protera Health. Call patients, explain the virtual back, neck, and joint care program, sign them up, and support them through the program. Needs 2+ years in a patient-facing or member-facing role. Up to $52,000 a year total pay.
About the Role
Protera Health's Care Navigator is often the first person a patient talks to and stays their main point of support through the program. You will reach eligible patients through inbound calls and outbound outreach (including AI-assisted outreach), screen them for fit, explain the program in plain language, guide them through signup, and keep them engaged until they finish.
What You Will Do
- Connect with eligible patients through inbound calls and outbound outreach
- Screen patients for program fit, explain the benefits, and guide them through signup
- Act as each patient's main point of contact from the first call to program completion
- Coordinate with physicians, physical therapists, and health coaches, and help patients keep appointments
- Escalate concerns that need clinical input
- Keep accurate documentation in CRM, EHR, scheduling, and phone systems
- Work toward outreach, enrollment, engagement, and service-quality goals
What You Need
- 2+ years in a patient-facing, member-facing, or care coordination role (such as patient access, health plan outreach, community health, or wellness coaching)
- Confident and engaging on the phone and video
- Comfortable with CRM and EHR systems and thorough documentation
- Organized and motivated by metrics
- A private, professional workspace and reliable high-speed internet
- Authorized to work in the United States
- Preferred: a clinical-adjacent background like medical assistant, CHW, or health coach, enrollment experience, or bilingual skills
Location & Pay
